Affidavit

An affidavit is a written statement of facts, sworn or affirmed before a commissioner for oaths or another authorised officer, used as evidence in a court application or proceeding.

In a road accident claim, an affidavit might support an application at filing a claim in court, confirm facts for an interim application, or set out evidence where a witness cannot attend in person. Because it is sworn, giving false information in it is a serious offence.

Preparing one carefully, with accurate dates and details, matters more than length, since the document stands as your formal evidence on record.

A lawyer helping with the claim will usually prepare the wording, but the facts inside it must come from you and must be true to the best of your knowledge.
